1. Change your user layout to Practicality.
2. Select 2 Column (Left Sidebar).
3. Enter Theme Customization.

Here's the complicated bits:

IN PRESENTATION: Change "Number of journal entries to show on the Recent Entries page" to 1.
IN FONTS: Paste this link in the Font Link at the bottom.
IN MODULES: Strip out everything except for "Link List", "Profile", and "Custom Text". Order them 0-1-2. Profile should have all checkboxes unchecked.
IN TEXT: Insert text like the sample below.

You can adjust it as you want; formatting as bold creates headers, and other formatting remains as normal.

I also adjusted "Text to show in a link to skip back/forward" through entries to "back" and "forward".

IN LINKS LIST: Create 3 links. Name them whatever you want, and link them somewhere. I chose "SUBJECT", "PROFILE", and "THREAD TRACKER". Just make sure you have 3 or less. It will attempt to make sense of it using 2, but I didn't test it.

IN CUSTOM CSS: Disable layout's stylesheet, and then paste in the following code:

I have put the part with the character profile picture in first; I also made sure it was super clear how to adjust it. You should be able to move it around however you want - be ready to tweak it a bit, but you should be able to figure it out with very little issues if you just play with it.
